  • Exclusion Deadline

    Friday, October 29, 2021
    Get out of the Settlement and retain your right to pursue your own lawsuit for these claims.  Get no cash payment from the Settlement.  You must complete and mail your request for exclusion form so that it is postmarked no later than Friday, October 29, 2021. You may also upload an Exclusion Form on this website under the Exclusion Section.
  • Objection Deadline

    Friday, November 12, 2021
    You may write to the Court about why you do, or do not, like the Settlement.  You must remain in the class to comment in support of or in opposition to the Settlement.  You must mail your objection(s) so that it/they are postmarked no later than Friday, November 12, 2021. You may also upload an Objection Form on this Website under Objection Form.
  • Claim Form Deadline

    Friday, October 29, 2021
    This is the only way to get a cash payment.  You must submit your Claim Form online no later than Friday, October 29, 2021, or mail your completed paper Claim Form so that it is postmarked no later than Friday, October 29, 2021.
  • Final Approval Hearing Date

    Tuesday, December 7, 2021
    Speak in Court to object to or comment on the fairness of the Settlement.  You may enter your appearance in Court either yourself or through an attorney at your own expense, if you so desire.
